Except that it says IC1: LM13600 and IC2: LM358 in the notes below the layout. That's what I wanted to adress. ;-) So the notes say something different than my comment. Just wanted to be sure that IC2 is the same as IC1.
Check all solder joints with a magnifying glass, you may have a solder bridge between the rows? Also make sure the in/out jacks are wired correctly? Good luck
IC3 has to be LM358, right? And IC1 and IC2 are LM13600?
